OUR PROGRAM MODEL
Client- centred approach
At Oasis Villages, we believe that every individual’s journey is unique and so should be the support they receive. Our practice model is rooted in a person-centred, strengths-based approach, designed to uplift and empower individuals living with developmental disabilities, mental health challenges, and Transitional Aged Youth (TAY) aged 16–24 as they navigate life transitions, challenges, and growth opportunities.
We do not offer generalized programming. Instead, we tailor every aspect of our services to reflect the distinct needs, goals, and strengths of the individuals we support, always in collaboration with their families, caregivers, and support networks.
Comfort, Culture, and Community
At Oasis Villages, we believe people thrive in environments where they feel recognized, respected, and truly welcomed. That’s why we create spaces that feel like home where culture, comfort, and identity are woven into everyday life.
We take time to learn what matters most to each individual, including the foods they enjoy, routines that feel familiar, the religions they practice and traditions that bring comfort. From culturally meaningful meals to personalized rooms and daily rhythms, we support individuals in ways that help them feel safe, grounded, and like themselves.
During short-term stays, our goal remains the same: to ensure every person experiences belonging, dignity, and care because home isn’t just a place, it’s a feeling.
A Plan Designed With You
At the core of our support model is the Individual Support Plan (ISP) a personalized roadmap created with the individual and their family or guardian, not just for them. Each ISP is designed to reflect the person’s unique strengths, needs, goals, and preferences, ensuring support feels meaningful, empowering, and aligned with their life vision.
Each ISP includes:
Individualized Goals: Whether someone is building daily living skills, improving mental wellness, transitioning toward greater independence, strengthening relationships, pursuing education, searching for meaningful employment, or increasing community involvement, goals are shaped by what truly matters most to the individual.
Collaborative Planning: We work closely with parents, guardians, and interdisciplinary professionals to ensure the plan reflects the individual’s full range of needs physical, emotional, behavioural, social, cultural, and developmental while remaining realistic and supportive.
Ongoing Evaluation: ISPs are living documents. We regularly review progress, celebrate achievements, and adjust strategies as needed, ensuring goals remain relevant, achievable, and responsive to changes in the individual’s life and circumstances.
Individualized Programming, Not One-Size-Fits-All
Our programs are designed with intentionality and flexibility because no two people are the same. From social and emotional development to vocational support and independent living skills, each service is tailored to:
• Address specific areas of concern identified by the individual and their family
• Support holistic growth and well-being
• Build meaningful connections and community engagement
• Empower individuals to take ownership of their goals
We recognize the vital role families and guardians play in the support process. That’s why we prioritize open communication, transparency, and collaboration at every step.
